The following information applies to all pharmacy students taking any rotation at the James A Haley VAH. Due to the new security requirements, even students who have just completed a rotation will need to complete the forms cited below.

Two Weeks Before

At least two weeks prior to starting your first day of your rotation, be sure to contact your assigned preceptor and verify that they know you are planning to take their rotation. Last minute changes to your schedule may not always have been communicated to the preceptor and this will insure that there are no surprises for either of you when you show up for your rotation.

One week before

The VA has new guidelines for students. Due to increased security requirements it will be necessary for all students to be finger printed and complete and sign some security agreements.

The requirement for the finger print and access forms are ONLY required on your first rotation at this VA. However you must check in with the pharmacy office on the first day of every rotation. These forms are:

  1. Statement of Committment and Understanding
  2. Finger Print Record Request Form
  3. Clinical Trainee Registration Form
  4. Request for background check memo
    • fill out the name, ssn, and dob sections of the form. The pharmacy office will complete the remainder of the form.
It will save time if you can print these form and bring them with you to your orientation on Day 1.

One week prior to your rotation, please contact the Pharmacy IT Security person, Mr. Nicholas Coblio, via phone at (813)978-5804 or email He will make sure that you are on the schedule for the student orientation. It is necessary to let our Human Resources staff know how many students to expect to finger print on the first day of your rotation and additional, unscheduled, individuals could cause delays in getting access to the computer system.

First Day

On the first day of your rotation, proceed directly to the pharmacy administrative office. DO NOT go to the Human Resource office. We will advise you when it is time to do so. The Pharmacy administrative offices are located on the second floor of Building 41. This building is to the left of the hospital emergency entrance, as you face the hospital building. The easiest way to find the Pharmacy Office is to follow the signs pointing to Human Resources. Human Resources is located on the first floor of the building.

Be sure to have your school ID with you. Human resources will not finger print you unless you have both your student ID and a driver's license or some other offical photo ID, such as a Passport. If you do NOT have a school ID you will need to bring with you two forms of Identification, such as a driver's license and passport.

Plan to arrive for a brief orientation at 8:30 AM on Monday, the first day of your rotation. If your rotation starts on an unusual cycle or you can not start on a Monday, please contact your preceptor to make other arrangements. Let Mr. Coblio know if you can not meet at the appointed time. Anyone arriving late will not be processed until the next day!

During the orientation, the four required forms will be collected. After the orientation you will be taken to Human Resources where you will be finger printed. Sometime, within 24 hours, you will be given access to the computer. At that point it will be necessary to complete two on-line training programs. After completion of these programs, you will be given your complete access to the clinical information system.

Parking

Parking at the James A Haley VA is problematic at best. Students are advised to park at the University Square Mall and take the VA shuttle bus to the hospital. The VA stops are clearly marked with a sign and are located next to the side entrace to the mall on the north east side of the mall, next to Steve & Barry's.

The shuttle stop for the main hospital is the second hospital stop and is located just under the over-head walk way to the USF medical school. From this stop, building 41, where the pharmacy administrative offices are located, is toward the water tower. As mentioned above, the easiest way to find the pharmacy would be to follow the signs to Human Resources.

The pharmacy office is on the second floor of the building in which Human Resources is located. This building is on the South side of the hospital. A new office building is being constructed in the parking lot indicated on the map (below). We will update the map as soon as a newer image becomes available.

The James A Haley VA is located at 13000 Bruce B. Downs Blvd, Tampa, FL 33612. It is located across from the USF Medical School and is connected to the medical school with an over-head walkway.
