Before The First Day
The VA requires that all individuals, including students and volunteers, who may have access to sensitive patient data be finger printed and have a background check. We have been told this must be done PRIOR to starting your rotation. It takes approximately 10 working days (two calendar weeks) to complete the background check.
In addition to the background check, individuals must complete two training courses BEFORE they can be given access to any sensistive patient information. The student must take the training and bring the signed certificate of completion with them when they report for their background check. The course work and required certificate were found in the public domain and are included below. You may want to check with individuals at the VA to determine if these forms are still valid.
In addition to the training the student must bring some forms with them when reporting to be finger printed. These forms are:
- Statement of Committment and Understanding
- Finger Print Record Request Form
- Only fill out the first page of the request form and only the top half of the second page. The remainder of the second page is for use by the security department.
- For the section labeled "Assignment over 120 days?" Circle "NO".
- For the section labeled "Providing direct patient services? Circle "YES".
- Clinical Trainee Registration Form
- Request for background check

- fill out the name, ssn, and dob sections of the form. The pharmacy office will complete the remainder of the form.
When reporting for the background check, Be sure to have your school ID with you. We are told that Human resources will not finger print you unless you have both your student ID and a driver's license or some other offical photo ID, such as a Passport. If you do NOT have a school ID you will need to bring with you two forms of Identification, such as a driver's license and passport.
Please note if you have recently changed your name, you will need to bring some legal documentation of this change!
topFirst Day
On the first day of your rotation, proceed directly to the pharmacy administrative office. DO NOT go to the Human Resource office. The Pharmacy administrative offices are located on the second floor of Building 41. This building is to the left of the hospital emergency entrance, as you face the hospital building. The easiest way to find the Pharmacy Office is to follow the signs pointing to Human Resources. Human Resources is located on the first floor of the building.
Plan to arrive for a brief orientation NO LATTER THAN 8:30 AM on Monday, the first day of your rotation. Anyone arriving late will not be processed that day. Depending upon scheduling, this could result in a significant delay to your having access to the computer. If your rotation starts on an unusual cycle or you can not start on a Monday, please contact your preceptor to make other arrangements. Let Dr. Coblio know if you can not meet at the appointed time.
Parking
Parking at the James A Haley VA is problematic at best. Students are advised to park at the University Square Mall and take the VA shuttle bus to the hospital. The VA stops are clearly marked with a sign and are located next to the side entrace to the mall on the north east side of the mall, next to Steve & Barry's.
The shuttle stop for the main hospital is the second hospital stop and is located just under the over-head walk way to the USF medical school. From this stop, building 41, where the pharmacy administrative offices are located, is toward the water tower. As mentioned above, the easiest way to find the pharmacy would be to follow the signs to Human Resources.
The pharmacy office is on the second floor of the building in which Human Resources is located. This building is on the South side of the hospital. A new office building is being constructed in the parking lot indicated on the map (below). We will update the map as soon as a newer image becomes available.
